Here’s what you will need:
– Application Form DS-11;
– Evidence of child’s U.S. Citizenship;
– ID of parents with photocopy;
– Evidence of relationship to child;
– A passport photo;
– Proof of travel;

Ready to get started?
Just follow these steps:

Step 1: Schedule an appointment:
Make an appointment at a regional passport agency by calling 1-877-487-2778.

Step 2: Complete Application Form DS-11:
Do not sign the application until instructed to do so by the passport agent.

Step 3: Gather Supporting Documents:
Provide evidence of U.S. citizenship such as a certified birth certificate or previously-issued, undamaged passport;
A copy of your itinerary or e-ticket can be used as proof of travel;
Include evidence of relationship to child such as a birth certificate or adoption decree.
Submit a standard passport photo taken within the last 6 months.

Step 4: Visit Regional Passport Agency:
Arrive early and present Appointment Confirmation Number and proof of travel;
Submit application and supporting documents when your queue number is called;
Pay fees by check, money order or credit card.
